POSITION SUMMARY
To manage the care of a defined caseload through the systematic process of assessment, planning, service coordination, monitoring and evaluation of services in order to achieve quality standards of care set forth by the company.
JOB REQUIREMENTS
Education
Graduate of school of nursing, Bachelor of Science in Nursing preferred
Experience
Relevant experience required; Prefer a minimum of three years nursing experience
Certificates & Licenses
Current licensure or permit by the state Board of Nursing, driver’s license, CPR certification, and automobile liability insurance
Other RequirementsKnowledge of nursing theory, practice, and principles. Ability to effectively implement and document nursing care and evaluate outcomes of care. Effective written and verbal communication and interaction skills.
