Description
The individual in this position will be responsible for coordinating patient care activities that conform to evidence-based practice and/or regulatory/payor requirements. The individual in this position is responsible for completing the discharge planning assessment. The individuals responsibilities will include, but not be limited to the following activities: a) completing the initial patient assessment of those patients meeting the high risk criteria; b)Respond to referrals resulting from admission screens within 24 hours of admission; c) Assure a viable discharge plan is put in place within 24-48 hours after admission; d) Identified needs will be addressed with the nursing staff, the case manager/UR nurse, and physician to develop an appropriate, safe discharge plan in a timely manner; e) Re-assessment/monitoring of discharge plan and any associated needs will be completed on an ongoing basis; f) Labor and delivery patients age 17 and under, will be assessed within 48 hours; g) NICU babies will be assessed within 72 hours after birth; h) General pediatric patients will be assessed within five days of admission; I) Pediatric patients in the pediatric intensive care will be assessed within 48 hours; j) The discharge planner will be responsible for making appropriate referrals on an ongoing basis; k) The discharge planner will be responsible for communicating and collaborating with physicians, their office staff and members of hospital ancillary departments (e.g., lab, pharmacy); k) The discharge planner will participate in daily reviews with the case manager, Director of Case Management, nursing staff and medical staff as needed with those patients who are identified to be high risk; L) participate in complex case review weekly at the LOS meeting.
Qualifications
Graduate of a School of Social Work accredited by the Council of Social
Work Education
Current licensure by the Texas State Board of Social Work Examiners
X Current BLS for health care providers preferred
Preferred Bachelor of Science in Social Work2 years experience in medical social work
