PRN LPN needed for Hospice Continuous Care
- Care of hospice patients in assisted living and private home settings.
- Provide hourly observation and monitoring with supportive documentation.
- Communicate any changes with family, Case Manager, and IDT members.
- Administer medications as ordered.
Requirements:
- Must have MO/KS LPN License.
- Hospice/Long term care experience a plus.
- Strong communication skills.
- Driver's license and reliable transportation.
- Ability to travel locally within assigned service areas in MO and KS.
