Hospice RN Case Manager

Care Alternatives Hospice embraces a holistic and personalized approach to delivering patient care. We are currently looking for (1) Per Diem RN Case Manager whose passion to deliver comprehensive hospice services will serve as a source of strength and support to our patients and families. This position will support our Delaware County service area.

Responsibilities:

·        Case management of hospice patients in long term care and home settings.

·        Development of individualized patient care plans.

·        Provision of direct care, education and support to patients and family members.

·        Management of medical regime as prescribed by attending physician(s).

·        Supervision of CNAs for your team.

Requirements:

• Current and valid PA Registered Nurse license.
• Completion of accredited nursing program.
• Minimum one (1) year experience in hospice, home care, or acute care setting.
• Understanding of hospice philosophy related to families in crisis and end of life issues.
• Strong interpersonal communication and documentation skills
• PC literate.
• Valid driver's license and reliable transportation.
• Must be able to travel locally within assigned territory.
