Job Summary
The Care Manager RN works in the Managed Long Term Care division providing case management guidance and coordinating care for Fidelis members needing long term care services in the home. This position collaborates with physicians and multidisciplinary team members, providers, members and families to provide the best care possible for each individual client.
Educational Requirements: Must possess a valid NYS RN license
Work Experience: Minimum of 1-3 years managed long term care and/or home care experience; 3-5 yeras preferred.
Technical Skills: Individual must be sensitive to client needs and address those needs as they arise. Ability to work within diverse communities, communicate effectively, follow up and close leads, and attention to detail are essential.
