Silverado is presently seeking a Bereavement Coordinator to join our Hospice team.
Qualifications:
- Education: High School Diploma required. Bachelor's degree preferred.
- Experience: Experience in clerical positions and in working as a member of a team.
- Other: Excellent oral, written and computer skills. Detail oriented with the ability to follow through, professional manner. Valid driver's license and auto insurance. Must be able to speak, read and write English.
Summary of Major Functions:
- The Bereavement Services Coordinator works in conjunction with the interdisciplinary team to administer a program of grief counseling and support services for survivors of patients served by Hospice.
- Distributes materials to the identified bereaved receiving services in accordance with the bereavement plan of care.
- Oversees bereavement follow-up by patient care staff and volunteers.
- Tracks bereavement assessments, plans and documentation provided by bereavement counselors.
- Will also facilitate community outreach meetings.
